Réponse immunitaire inappropriée envers une infection (souvent sévère ou disséminée, ex. passage dans la circulation sanguine) qui produit la défaillance d'un ou plusieurs organes : insuf. respiratoire ou rénale aiguë, confusion ou coma, instabilité cardiovasculaire, atteinte hépatique, coagulopathie.

Le sepsis peut progresser rapidement vers le choc septique, où il s'accompagne d'une hypotension qui ne répond pas au remplissage vasculaire.

L'administration rapide d'antibiotiques, le support hémodynamique (remplissage, amines vasoactives si besoin) et respiratoire, et le contrôle de la source d'infection restent les piliers de sa prise en charge.

Mortalité élevée : 20 - 30%, malgré tout.

Causes :

Production exacerbée de cytokines et médiateurs inflammatoires face à une infection => lésions microvasculaires diffuses, vasodilatation générale.

Infections les + fréquentes à l'origine : pulmonaires, urinaires et infections intra-abdominales.

Bacilles Gram(-) et Staphylocoques surtout, mais des nombreux autres microorganismes sont possibles.

Traitements

Liste des traitements : 1ème ligne
  • Priorité !! A initier très rapidement !!
    Antibiothérapie empirique de 1er choix si pas de foyer évident et si pas d'allergie connue aux céphalosporines. A initier très rapidement : le temps écoulé jusqu'à la 1ère dose est directement corrélée à la mortalité (Kumar 2006, Peltan 2019). En monothérapie, de préférence, sauf si facteurs de risque de germe résistant (voir alternatives plus bas) (Sjövall 2017, Surviving Sepsis 2021)
  • Systématique si hypoTA
    Si hypoTA, en systématique. 0,5 - 1 L d'un cristaloïde (Ringer Lactate préféré, serum physiologique sinon), à passer rapidement et à répéter si la TA ne remonte pas, jusqu'à 30 ml/Kg dans les premières 3 heures (Surviving Sepsis 2021).
    Éviter de perfuser des très gros volumes (CLASSIC 2022, CLOVERS 2023)
  • Patients avec sepsis sévère ou choc septique
    Chez les patients en sepsis sévère ou choc septique. 50 mg IV/6h x 5 - 7 jours. Les études montrent une résolution plus rapide du sepsis et choc et une possible réduction de la mortalité finale (CRICS-TRIGGERSEP 2018, Annane 2019, Surviving Sepsis 2021)
Traitements alternatifs
  • Si hypoTA qui ne répond pas rapidement au remplissage
    Si la TA ne répond pas rapidement au remplissage. Une stratégie d'initiation précoce de la noradrénaline si hypoTA persistente semble produire des meilleurs résultats qu'insister sur le remplissage ou attendre (CENSER 2019, Ahn 2024)
  • Si allergie aux céphalosporines
    La levofloxacine est une alternative, si allergie aux bêta-lactamines, en Tt. empirique. A combiner avec vancomycine ou linézolide si besoin de couvrir les Cocci Gram(+)
  • Si suspicion de : anaérobies, Pseudomonas
    Antibiotique empirique alternatif, si antécédents ou facteurs de risque d'infection à anaérobies (infection intra-abdominale, infections nécrosantes des tissus mous ...) ou à Pseudomonas. Peut être combinée avec la ciprofloxacine, pour une meilleure activité
Cas particuliers
  • Si antécédents ou facteurs de risque de bactérie multirésistante
    Antibiotique empirique alternatif, si antécédents ou facteurs de risque d'infection à bactérie multirésistante ou Pseudomonas
  • Si risque d'infection à Saph. méticillin-résistant (SARM)
    Ou linézolide, toujours en association à un autre antibiotique de large spectre, si risque d'infection à Staphyloccoque résistant à la meticilline (antécédents, infection cutanée, cathéter, prothèse ...)
  • En association à la noradrénaline, si hypoTA réfractaire
    En association à la noradrénaline, quand celle-ci ne suffit pas à obtenir une amélioration suffisante ou il existe une défaillance de la pompe cardiaque associée
  • Alternative à l'association noradrénaline + dobutamine
    Alternative à l'association noradrénaline + dobutamine, dans les cas où il existe une défaillance de la pompe cardiaque ou la noradrénaline seule ne suffit pas. Alternative à la noradrénaline si celle-ci non disponible
  • Attention ! Traitement à risque
    Risque élévè de toxicité rénale dans le sepsis / choc
    A éviter ! sauf si bactérie multirésistante documentée ou fortement suspecte. L'ajout d'un aminoside aux bêta-lactamines n'améliore pas l'évolution clinique ni la mortalité et majore le risque d'insuf. rénale aiguë grave (Paul 2014)

    Peut aider à différentier les infections bactérienne sévères des réactions inflammatoires systémiques d'autre cause, notamment celles en réponse à une infection virale

    Si élevé, argument en faveur de sepsis sévère. Mais attention, il peut être élevé aussi dans d'autres situations d'origine non infectieux (toute hypotension et hypoxémie cellulaire) et être normal dans une sepsis pas severe, au stade initial
