Le cholestérol est utilisé pour mesurer indirectement les taux de lipoprotéines dans le sang. Plusieurs lipoprotéines sont comptées dans le cholestérol total : LDL, HDL, VLDL et en petite quantité, IDL et Lp(a).

Les lipoprotéines contenant de l'apoB - VLDL, LDL, IDL, et Lp(a), dont les LDL sont le type principal - sont directement impliquées dans le développement de l'athérosclérose : de petite taille, elles traversent la barrière endothéliale, si endommagée, et s'accumulent dans la matrix de la paroi artérielle.

Le cholestérol non-HDL est une mesure plus précise des lipoprotéines apoB et du risque cardiovasculaire associé aux lipides. Les objectifs thérapeutiques sont à adapter au risque individuel.

Type de prélèvement :

Sang veineux, traditionalement à jeun, mais peu de variations avec les taux prélevés sans jeun.

Valeurs de référence

  • Cholestérol total : valeurs normales

    < 5 mmol/L

    Soit < 200 mg/dL

  • Cholestérol LDL : taux recommandés, patients SANS évènement cardiovasculaire

    Risque bas : < 3 mmol/L (116 mg/dL)
    Modéré : < 2,6 mmol/L (100 mg/dL)
    Haut* : < 1,8 mmol/L (70 mg/dL)

    * Obtenir au moins une réduction de > 50% par rapport aux taux avant Tt.
    Risque CV selon SCORE2 : < 2% = risque bas, 2-10% = modéré, 10-20% = haut.

  • Cholestérol LDL : taux recommandés, patients AVEC évènement cardiovasculaire

    Risque très haut : < 1,4 mmol/L (55 mg/dL)
    Risque extrème : < 1,0 mmol/L (40 mg/dL)

    Tout évènement cardiovasculaire, ancien ou récent = risque très haut. Si évènement cardiovasculaire récurrent malgré traitement sous statines ou atteinte polyvasculaire (ex. coronaire + AOMI) = risque extrème.

  • Cholestérol HDL : taux recommandés

    > 1 mmol/L

    Soit > 40 mg/dL

  • Cholestérol non-HDL : taux recommandés selon niveau de risque cardiovasculaire

    Modéré : < 3,4 mmol/L (130 mg/dL)
    Haut : < 2,6 mmol/L (100 mg/dL)
    Très haut : < 2,2 mmol/L (85 mg/dL)

    Même classification du risque CV que pour le cholestérol-LDL.

Précautions

  • A interpréter selon l'histoire clinique et l'ensemble des facteurs de risque
    Les recommandations sont très différentes en cas de :
    1) prévention primaire => taux à interpréter en fonction du risque cardiovasculaire globale : voir SCORE2 et SCORE2-OP.
    2) prévention secondaire, après évènement cardiovasculaire : traitement systématique, objectifs plus stricts
