Élimination d'un thrombus frais d'une grosse artère intracrânienne, pulmonaire ou autre, avec un dispositif mécanique de retrait du caillot (habituellement un "stent retriever") via un cathéter intravasculaire.

Très efficace dans la phase hyper-aiguë d'un infarctus cérébral, quand une obstruction d'une grosse artère est identifiée à l'imagerie (angio-IRM ou angio-scanner).

Technique complexe, nécessite d'une équipe de radiologie interventionnelle bien entraînée. En revanche, elle comporte un moindre risque hémorragique et a moins de contre-indications que la thrombolyse avec des médicaments fibrinolytiques.

Mécanisme d'action :

Procédure interventionnelle percutanée, de radiologie vasculaire. Après ponction de l'artère fémoral, un cathéter est introduit et améné jusqu'à l'artère obstruite, un stent spécial est passé à travers le caillot et déployé pour ensuite le rétirer.

Doses habituelles

  • Thrombectomie

    A faire en toute urgence

    1 seule fois ;

    Dans les toutes premières heures après l'apparition des symptômes, idéalement dans les 4-6 h, mais possible plus tardivement si l'imagérie cérébrale montre l'existance d'une zones de pénombre ischémique (mismatch)

A surveiller

  • Risque de saignement à niveau de la poction artérielle
    Effet indésirable rare : CardioVasc :
    Pansement compressif. A surveiller
    Pansement compressif nécessaire. A surveiller fréquement les premières 24-48h. Surveiller aussi la possible formation d'un pseudo-anévrisme artériel
  • Risque de transformation hémorragique de l'infarctus après la révascularization
    Effet indésirable fréquent : Neuro :
    Surtout si infarctus étendu, mais bénéfice global
    Le plus étendu l'infarctus cérébral, le plus élévé le risque de transformation hémorragique. La thrombectomie peut être donc contre-indiqué dans les très gros infarctus. Cependant, des études ont montré que la thrombectomie obtient un bénéfice global malgré le surcroit de transformations hémorragiques, même dans les infarctus très grands (ANGEL-ASPECT 2023, LASTE 2024)

Précautions

  • Doit être réalisée dans un centre hospitalier disposant d'une unité spécialisée
    Procédure :
    Le centre doit disposer d'une unité de soins intensifs de neurologie vasculaire et d’un centre de neuroradiologie interventionnelle autorisées, et de professionnels entraînés (neurologue, neuro-radiologue, anesthésiste-réanimateur, infirmiers)

Indications

  • En toute urgence, si thrombose d'une artère intracraniênne (grosse ou moyenne) retrouvée à l'imagerie ou suspectée. A faire dans les 6h après le debut des symptômes, ou jusqu'à 24h si l'imagerie cérébrale montre une zone d'hypoperfusion récupérable (mismatch), dans un service hospitalier spécialisé. Peut s'associer à la fibrinolyse. (HERMES 2016, Recos Europe ESO 2019, Recos USA AHA/ASA 2026)
  • Autres indications :
  • Si embolie pulmonaire grave et fibrinolyse contre-indiquée ou réalisée sans succès. Alternative : embolectomie chirurgicale
