Dans ce type de régimes une partie importante des besoins énergétiques normalement apportée par les glucides (50% du total habituellement) est substituée par des lipides et des protéines.
Ainsi, sont fortement réduits : tout produit issu des céreaux (pain, pâtes, pâtisseries ...) ou avec sucre ajouté (chocolat, sodas ...), les pommes de terre, légumineuses et fruits. En échange, on augmente les légumes non féculentes, les protéines (oeufs, poisson, viandes, produits laitiers), noix, graines et fruits acides.
Surtout utilisés pour perdre du poids (ex. regimes Atkins et Ferriss) ces régimes pourraient avoir aussi des bénéfices dans le sd. de l'intestin irritable et le sd. métabolique (voir références)
Mécanisme d'action :
Il s'agit habituellement de :
- Réduire les apport caloriques, dans le but d'obtenir une perte pondérale, ou de
- Restreindre certains glucides suite à des intolérances digestives (gluten, oligosaccharides fermentescibles ...)

Une étude a montré qu'un régime pauvre en carbohydrates est associé aussi à une amélioration significative des symptômes, supérieure aux médicaments (CARIBS 2024). Il peut être plus facile à suivre qu'un régime FODMAP
