Médicaments à arrêter en cas d'Insuffisance Rénale Aiguë (IRA)

En cas d'insuf. rénale aigue il faut envisager l'arrêt temporaire de plusieurs classes de médicaments d'usage fréquent, car :
- ils peuvent aggraver l'atteinte rénale, ou
- s'accumulent rapidement avec risque d'effets indésirables graves.
L'idéal est de suspendre ces médicaments rapidement au début de l'insuf. rénale.
Ne pas oublier de les reprendre dès que la fonction rénale se normalise, s'ils sont importants au long cours.
Attention : cette liste n'est pas exhaustive.

Dépister les médicaments d'usage fréquent qui sont à arrêter en cas d'insuf. rénale aigue.

jusqu'à que la fonction rénale se normalise. N'attendez pas à que l'insuf. rénale aigue progresse
Rappelez vous : les formules MDRD, CKD-EPI, etc, sont valables seulement en cas d'insuf. rénale chronique stable, et non dans l'IRA

A la résolution de l'insuf. rénale aigue, s'ils sont des médicaments à maintenir au long cours

En cas d'insuf. rénale aiguë, il est important de suspendre plusieurs médicaments, soit parce qu'ils peuvent aggraver l'IRA, soit parce qu'ils s'accumulent rapidement dans cette situation, avec un risque d'effet indésirable grave

La maladie rénale chronique fragilise les reins et comporte un risque majoré faire des épisodes d'insuffisance rénale aiguë en plus. Les médicaments potentiellement néphrotoxiques et ceux qui dépendent grandement de l'élimination rénale sont à adapter ou à arrêter
